Who You Are

We are seeking an experienced and motivated Social Worker to join our Reablement Team. The ideal candidate will be an experienced Social Worker with a strong understanding of adult social care legislation, safeguarding, and reablement, who can work effectively within a supportive team environment to deliver positive outcomes for adults and their carers.

What the job involves

The role involves undertaking Care Act assessments, support planning, duty responsibilities, and responding to safeguarding concerns and enquiries. Working within a diverse multicultural community, the post holder will promote strengths-based practice, identify reablement opportunities, and make appropriate referrals to therapy services and community resources to maximise independence and reduce reliance on long-term support. The successful candidate will attend funding panels, presenting assessments and recommendations to ensure care and support arrangements are proportionate, person-centred, and outcome focused.

Skills

  • Sound knowledge of the Care Act 2014, Mental Capacity Act 2005
  • Experience managing safeguarding concerns
  • Strong assessment and support planning skills
  • Ability to manage a varied caseload
  • Experience working within a multidisciplinary team
  • Skilled in reablement and promoting independence
  • Excellent communication and interpersonal skills
