Who You Are
We are seeking an experienced and motivated Social Worker to join our Reablement Team. The ideal candidate will have a strong understanding of adult social care legislation, safeguarding, and reablement, and can work effectively within a supportive team environment to deliver positive outcomes for adults and their carers.
What the job involves
In this role, you will manage a varied and fast-paced caseload while working collaboratively within a multidisciplinary team. Responsibilities include undertaking Care Act assessments, support planning, duty responsibilities, and responding to safeguarding concerns. You will work within a diverse multicultural community, promote strengths-based practice, and present assessments and recommendations at funding panels to ensure care is outcome-focused.
Skills
- Knowledge of Care Act 2014 & Mental Capacity Act 2005
- Experience managing safeguarding concerns
- Strong assessment and planning skills
