What the Job Involves
This role involves delivering a strength-based, person-centered assessment and support planning service. You will be responsible for facilitating safe hospital discharges using the Discharge to Assess Model and the Home First Policy. Your duties will include completing discharge support plans, conducting mental capacity assessments, safeguarding, planning complex discharges, providing advice and advocacy, facilitating multidisciplinary meetings, and being part of the weekly duty work rotation. You will work in collaboration with various health and social care agencies to provide coordinated, high-quality outcomes for those needing support.
