Who You Are
You are a committed individual with a caring nature, willing to work in a dynamic and fast-changing service environment. You possess good observation and recording skills and are flexible and focused in your approach. You should have the ability to train towards achieving the relevant Health and Social Care qualifications. Being able to speak multiple languages, particularly Welsh and community languages, is an advantage. You must hold a full clean driving license and have access to a car.
What the Job Involves
As a member of the Community Resource Team (CRT) Homecare in Cardiff, you will support clients to achieve their reablement and wellbeing goals, working alongside health professionals. You will operate under a service delivery plan, with tasks allocated via a mobile device, working shifts according to a rota. Various rotas and hours are available, including evening and weekend work. You will play a pivotal role in preventing unnecessary hospital admissions and facilitating hospital discharges, enabling clients to live independently at home.
